The ABCs of an ACO

Kirby Medical Group is participating in a new care coordination program in Medicare! As an Accountable Care Organization (ACO) and member of the Illinois Rural Community Care Organization (IRCCO), Kirby Medical Group physicians have agreed to work together with Medicare to provide high-quality care at the right time – and in the right setting. Benefits include:
  • Patient centered medical care
  • Coordinated care teams
  • Fewer medical forms to complete
  • Excellent communication with caregivers, physicians and specialists

Please note: An ACO is not a Medicare Advantage plan or an HMO plan. While Kirby Medical Group may recommend that you see particular doctors for your specific health needs, it remains your choice which doctors you see and which hospitals you visit.

If you have questions or concerns, feel free to speak with Kirby Medical Group at any time. You can also visit www.iruralhealth.org or call 1-800-MEDICARE. (TTY users, please call 1-877-486-2048.)
