Only $25- A Great Savings!!!
$20 from each clinic physical will be donated to the school or Athletic Club of athlete
No reservations required. Physicals provided on a first-come, first-served basis. All student athletes must have the signature of their parent or guardian on their sports physical form to participate. Students will also need a Consent to Treat signed by a parent or guardian. All area athletes are eligible.
